Admission Tittle:Community Health Officers Training Programme.

Entry Requirements:
A) Basic Educational Qualification:

• West African School Certificate/GCE '0' Level (WAEC) or Senior Secondary School Certificate (NECO) with five (5) credits as follows: English Language , Mathematic  ,Biology or Health Sciences and any two (2) of  the following Subjects: Chemistry, Physic, Geography, Food and Nutrition and one (1) from Economics, Accounting, Govemment, Commerce, Agricultural Science, Literature in English and Book Keeping. Certificates presented should be at not more than two (2) sittings.

B. Professional Qualifications and Experience :
In additional to the qualifications above, candidates must possess any of the following professional qualifications and  experience for eligibility to apply:

• Diploma in Community Health (DCH) with not less than two years post Qualification expenence (up to September  2013).
•  Community Health Extension Workers (CHEW) with 5 years experience plus Abridged Certificate.
• Applicants must be registered and possess a Valid practice license.

Written Test and Oral Interview selection for admission will be based on candidates' performance at a competitive written test and oral interview to be conducted on Monday, 10th and Tuesday, 11th June, 2013 respectively .

• Application Guildlines Pay a non-refundable application fee of Six Thousand Naira (N6,OOO) only, at any branch of First Bank of Nigeria Plc.

                       (b) Bank teller (evidence of payment of application fee)
                       (c) Valid and current Practice License.
                       (d) WAEC or NECO Certificates, or Master-list(s) of the result (s)
                        (e) CHEW Certificate and Abridged Certificate (if applicable)
                         (f) DCH Certificate (if applicable)
                        (g) Three (s) recommendation letters
                        (h) Evidence of change of name (if applicable).
                        (i) Birth certificate or age declaration.
