University Of Port Harcourt Teaching Hospital Admission into School Of Medical Record for 2014/2015 Academic Session - Applications are invited from suitably qualified candidates for admission into the school of Medical Records for the following programmes:

Programmes

A.)  National Diploma

• Duration: 2 years programme leading to the award of National Diploma in Health Information Management (HIM)

Requirements

• 5 credit level passes in not more than two sitting of the WASC or NECO or GCE in Mathematics, English Language., and any three of Biology. Chemistry; Health Science, Physics.

B.) Higher National Diploma

Duration: 2 years programme leading to award of Higher National Diploma in Health Information Management (HIM).

Requirements

• 5 credit level passes in not more than two sitting of the WASC or NECO of GCE O level Examinations in Mathematics, English Language and any three of Biology, Chemistry Health Science, Physics. In addition, prospective applicants should possess any of the following.

i.) National Diploma in Health Information Management with passes at not lower than lower Credit and have completed one year of industrial work experience in a recognised hospital
ii.) Health Records Technician certificate with a minimum of lower credit pass, one year of industrial Work experience in a recognized hospital and minimum of five years working experience

Method Of Application

• Candidates should pay a non-refundable Fee of N5000 (five thousand Naira) only to Sterling Bank (UPTH Branch) and present the teller to the cashier at the office of the Assistant Director, (Finance) for the issuance of an official receipt and on presentation of evidence of such payment, obtain an application form from the office of the Assistant Director of Administration, Clinical Services & Training
• Completed application forms with photocopies of all relevant documents are to reach the office of the Assistant Director of Administration, Clinical Services & Training before the application closing date

Selection Interview

• Entrance Examination will hold in the school of post Basic Nursing Class Room 2 UPTH on March 6th 2014 at 10.00am prompt
• Candidates are to bring along original copies of their certificates and writing materials
• Mobile phones are not allowed into the examination hail.
• Please note that candidates are to make necessary arrangements for their accommodation and feeding throughout the duration of the interview
