Job Title: Hospital Pharmacist

Location: Ikeja, Lagos

Job Summary

  • We are recruiting to fill the position of Hospital Pharmacist.
  • The ideal candidate will be responsible for drug distribution, provision of clinical services, and will be required to perform other functions as stated below:

Responsibilities

  • Always maintain the drug stock in the hospital. This includes storing medications safely, and monitor availability (low in stock) to prevent out-of-stock.
  • Check prescriptions for appropriateness and proper dosage and communicate with prescribers in situations where dosage adjustment is required, or an alternative is suggested.
  • Provide advice on the dosage of medicines and the most appropriate form of medication to healthcare professionals.
  • Participate in ward rounds, multidisciplinary team meetings, and other clinical pharmacy activities such as medication reconciliation, antimicrobial stewardship, adverse event monitoring, etc.
  • Dispense prescribed medication to patients, advise them on the indications, contraindications, drug-drug interactions or food-drug interactions, side effects, and dosage, and state the need for compliance to the dosing regimen when necessary.
  • Compound of extemporaneous preparations.
  • Maintain a database of patient drug history, and proper inventory control of medicines available in the pharmacy.
  • Supervise the work of less experienced and less qualified staff on duty.
  • Attend to all drug information requests from other members of the health care team, patients, and the public.
  • Keep up to date with and contribute to research and development.
  • Write guidelines for drug use within the hospital and implement hospital regulations.

Minimum Requirements
Academic Qualification:

  • Minimum of Bachelor (B.Pharm) in Pharmacy from a recognized institution .

License and Certification:

  • Valid pharmacy license from the Pharmacy Council of Nigeria (PCN).

Experience:

  • Minimum of 3 years of Hospital Pharmacy experience in a well-recognized hospital.

Required Skills:
