Applications are invited from suitably qualified candidates for admission into the School of Health Information Management, Nnamdi Azikiwe University Teaching Hospital, Nnewi, Anambra State for the 2012/2013 Academic Session. The programme is in two parts. Port I and II leading to the award of the National and Higher National Diploma of the Health Records Officers Registration Board of Nigeria.
Entry Qualifications:
Part I Programme (ND)
- Prospective candidates must possess a minimum qualification of Senior Secondary School Certificate or its equivalent with credit passes in five (5) subjects. The subjects to be credited must include: English Language, Mathematics, Biology or Health Science, and any other two subjects.
- Possession of the National Certificate in Health Records Sciences will be of added advantage.
Part II Programme (HND)
- Candidates who hold the National Diploma (ND) in Health Information Management and also met the normal entry requirements as stipulated above.
Matriculation:
Any student admitted to any part of the Programme for the first time to read for either the ND or HND will be required to matriculate at a formal ceremony and sign the register of Matriculating students.
Conditions For Admission:
Selection of candidates for the Parts I and II programme will be based on the ability to pass the competitive selection entrance examinations and admission interview. For the Part I, candidates are expected to answer questions in
- Use of English
- Mathematics
- Biology or Health science
Questions will be based on the SSSC/GCE/NECO Syllabus. Candidates for the Part II (HND) programme will be expected to answer professional questions on the ND scheme.
Accommodation:
The course is non residential as students are advised to arrange for their accommodation but however, limited accommodation available will be on first come first served basis. Students are to pay the economic cost for the apartment.

and marked for the attention of the Course- Coordinator, School of Health Information with a non refundable application fee of N5,000 (Five thousand Naira Only) in Certified Bank Draft Payable to Nnamdi Azikiwe University Teaching Hospital, (NAUTH) Nnewi,
- Candidates can also come personally to the school with Certified Bank Draft or cash to be paid at the Zenith Bank in the hospital (NAUTH Nnewi), dully receipted and collect the admission form thereafter.
- The closing date for the sale and submission of the part I and part II programme forms shall be Wednesday 28th November, 201 2
- Application duly completed must be returned to the coordinator on or before the closing date.
- The date for the entrance and oral interview is scheduled for Thursday 13th — Friday 14th December, 2012.
- Prospective candidates should come along with the Original Copies of their Certificates for interview.
