Job Description
- Community health worker (CHW) acts as a vital link between health services and local communities, providing outreach, education, advocacy, and basic health services to improve health outcomes.
- They build trust within neighborhoods to bridge gaps in care, often focusing on disease prevention, chronic condition management, and connecting residents to social resources, such as housing or food assistance.
Key Responsibilities and Functions
- Health Education & Promotion: Providing information on nutrition, disease prevention, and sanitation.
- Case Management & Advocacy: Helping individuals navigate complex healthcare systems and advocating for patient needs.
- Direct Support Services: Conducting home visits, offering informal counseling, and providing health screenings.
- Bridging Cultural Gaps: Providing culturally relevant information and care, as they often share the same background, language, and life experiences as the population they serve.
- Improving Access: Connecting community members with health and social services.
Salary
N100,000 - N300,000 monthly.
