Job Title: Hospital Pharmacist

Location: Abuja

Job Summary

  • A Hospital Pharmacist is responsible for ensuring the safe, effective, and appropriate use of medications within a hospital or healthcare facility.
  • They work closely with physicians, nurses, and other healthcare professionals to optimize patient care, prevent medication errors, monitor drug therapy, and provide pharmaceutical expertise for both inpatient and outpatient services.

Key Responsibilities
Medication Management:

  • Review and verify medication prescriptions for accuracy, appropriateness, and safety.
  • Dispense medications according to legal and professional standards.
  • Prepare and compound medications when required, including sterile preparations.
  • Ensure correct storage, handling, and distribution of pharmaceutical products.
  • Monitor medication inventories and prevent stock shortages.

Clinical Pharmacy Services:

  • Participate in patient care rounds with healthcare teams.
  • Assess patient medication regimens and recommend therapy adjustments.
  • Monitor patient responses to drug therapy and identify adverse drug reactions.
  • Evaluate potential drug interactions, contraindications, and allergies.
  • Support evidence-based medication decisions.

Patient Counseling and Education:

  • Educate patients and caregivers about medication use, dosage, side effects, and precautions.
  • Provide discharge medication counseling.
  • Promote medication adherence and safe medication practices.
  • Answer patient questions regarding prescriptions and treatment plans.

Drug Information Services:

  • Serve as a resource for healthcare professionals on medication-related issues.
  • Research and provide information on new drugs, treatment guidelines, and pharmacotherapy.
  • Develop medication-use policies and protocols.
  • Support formulary management and drug evaluation activities.

Medication Safety and Quality Assurance:

  • Implement strategies to reduce medication errors.
  • Report and investigate adverse drug events.
  • Participate in quality improvement initiatives.
  • Ensure compliance with hospital policies and regulatory requirements.
  • Conduct medication-use evaluations and audits.

Inventory and Supply Chain Management:

  • Maintain adequate pharmaceutical stock levels.
  • Monitor expiration dates and ensure proper stock rotation.
  • Coordinate procurement of medications and medical supplies.
  • Manage controlled substances according to legal requirements.
  • Maintain accurate inventory records.

Regulatory Compliance:

  • Comply with pharmacy laws, healthcare regulations, and accreditation standards.
  • Maintain records of medication dispensing and controlled substances.
  • Participate in inspections and audits.
  • Ensure adherence to infection control and safety protocols.

Administrative Duties:

  • Supervise pharmacy technicians and support staff.
  • Assist in developing departmental policies and procedures.
  • Participate in budgeting and resource planning.
  • Contribute to staff training and professional development programs.

Requirements

  • Bachelor of Pharmacy (B.Pharm), or equivalent pharmacy degree.
  • Completion of internship or residency training where applicable.
  • Valid pharmacist license/registration with the relevant pharmacy regulatory authority.

Experience:

  • 4-5 years experience in hospital pharmacy practice is preferred.
  • Familiarity with clinical pharmacy services and medication management systems.
