Job Title: Medical Record Clerk

Location: Ikeja, Lagos
Employment Type: Full-time

Job Description

• Gather patient demographic and personal information.
• Distribute medical charts to the appropriate department of the clinic.
• Assist with department audits and investigation.
• Issue medical files to persons and agencies according to laws and regulations.
• Maintain up to date quality and accurate record by following clinical procedures for ALL patients, Medications, Equipment, Supplies, and Reagents.
• Retrieve medical records by following chart-out procedures; documenting reasons chart cannot be retrieved for statistical and follow up purpose.
• Ensure patient chart, paperwork and report are completed in an accurate and timely manner.
• Deliver charts to assigned area of the hospital by following established routing procedures.
• Ensure the accuracy and protect the integrity and confidentiality of all patients record.
• Align all electronics record arrangement with physical (hard copy) records.
• Managing and updating patient’s next appointment in line with treatment plan as may be recommended by doctor(s) on duty.

Requirements

• Candidates should possess a Bachelor's Degree with 2 - 3 years work experience.
