Job Title: State Team Lead / Integrated Health and Support Specialist

Job Summary
The state program manager is responsible for direct management and coordination of all project activities in the state, in line with project implementation plan.
S/He ensures that all project activities in the state meet effective programming standards and best practices established by USAID and the Nigerian Government especially the National Standards for Improving the Quality of Life of Vulnerable Children in Nigeria.
S/He ensures that services provided to vulnerable children are needs based and in line with the National service standard and the PEPFAR guidelines.
S/He provides support to stakeholders, strengthens technical capacity, carry out advocacy, supports community improvement team, provides timely update on grant work plan and coordinate state project activities to ensure successful implementation of all projects.
Work with technical advisors to ensure the standard of care is in line with national and international standards and in line with project objectives and deliverables.
Responsible for HIV care and support services for the OVC and their household
Coordinate with other stakeholders to ensure enrollment and provision of requisite services to all HIV-related subpopulations enrolled in the OVC project and liaise with the treatment implementing partners to ensure all CLHIV are identified and enrolled in the OVC care.
She is part of the managing team of enrolled beneficiaries and has the duty to ensure prompt access to care, especially during emergencies as well as ensure HIV Care service provision at the household is designed to demonstrate attribution to the UNAIDS 95 95 95 epidemic control goal.
Specific Job Responsibilities
Provide leadership and coordination to transitioned and new CSO partners to implement THRIVE based on the project design and objectives.
Facilitate a working environment of mutual trust, respect, and care among local project team.
Encourage effective teamwork, with case managers and CCWs supporting each other.
Conduct regular meetings to ensure issues are raised and addressed and to encourage reflection on project progress and learning for improvement.
Oversee the implementation of planned activities to ensure targets are achieved:
Ensure that project activities are carried out in a timely manner in line with USAID approved work plan, and that the project achieves results in line with its annual approved work plan, and that the project achieves results in line with its annual PEPFAR targets in each program area.
Ensure each CSOs has a clear weekly work-plan aligned with approved monthly work plan and budget.
Conduct monitoring visits to project activities to assess progress against targets and to providecoaching and direction to activities in areas where improvements are needed.
Act as the initial trouble-shooter when difficulties in implementation are identified; meet with
necessary project participants, listen to challenges and actively identify solutions such that projects can continue as needed.
Submit timely, accurate and professional reports that meet donor requirements. Prepare reports to be submitted for donors, State and local government offices.
Monitor local context and report on how changes in context may affect program activities. This includes changes in the security environment.
Closely collaborate and network with government stakeholders and others to ensure transition and sustainability of the program, continued partnership and sharing best practices in various fields.
Co-ordinate with other USAID partners or organizations in the area to ensure complimentaryservice provision for the project beneficiaries.
Promptly escalate program implementation challenges to supervisor and project management team
Under the direction of the Integrated Health and Support Advisor, collaborate closely with theTHRIVE team to roll out and implement an integrated and holistic community-based response to HIV /AIDs towards scaling up access to HIV prevention and care services for vulnerable children and their caregivers
Work with the Treatment partners and coordinate bidirectional patient referral from facility tocommunity and vice versa.
Obtains patient line list regularly and ensure data synergy with the treatment partners along allcustom indicators.
Lead the process of monitoring of quality HIV care at the household and ensure optimal drugadherence and viral load suppression
Document OVC project attribution to the care and treatment partners overall achievement along the 95 95 95 epidemic control cascades.
Work closely with the SMOH, SMWASD, SACA and HIV TWGs in the state assigned in identifying and capitalizing on leverage points for improved linkages and referral coordination at the community level and for Integration of HIV Prevention Care & Support elements into other OVC areas project activities and vice versa
Perform other tasks assigned to him/her by the supervisor or the THRIVE Project Management Team.
Qualifications and Skills
MBBS with a minimum of 5-7 years of relevant experience in implementing HIV-TB Treatmentand Prevention Program, technical experience with community level HIV prevention, Care &Support programs:
Additional Qualification - Master of Science and / or MPH will be an addedadvantage
Demonstrated ability to work effectively with government and civil society partners andcommunities as well as transfer knowledge to diverse audiences through workshops, training and meetings using participatory methods and adult learning principles
Experience in supporting HIV Testing and Counselling programs in health facilities, mobile units and community outreaches
Good knowledge of Government of Nigeria HIV Care and Treatment guidelines (Pediatric&Adult), PMTCT, TB, other STIs and experience working with Ministry of Health and otherrelevant entitles
Knowledge of case management and national standards of care for orphans and vulnerable children
Previous 5 years’ experience of managing sub-award recipients
Familiarity with PEPFAR-OVC programming and principles
Excellent English oral and written communication skills required
Excellent analytic, organizational and computer skills; skilled In MS Office including Word, Excel, Outlook and PowerPoint.
KeyWorking Relationships:
Supervisory: Directly supervises the program specialist and officers, thematic areas specialist, Office
Administration officer. Also coordinate the operational plans of project advisors.
Internal: Director of programs, Thematic Advisors
External: CSO Partners, State government agencies, other implementing partners.
