Job Purpose
The CCMP Associate will support the implementation of Community Case Management Program (CCMP) activities for children, adolescents, and households affected by HIV in Kaduna State. The position will contribute to improved health, nutrition, and HIV outcomes, with specific focus on achieving progress across the 1st, 2nd, and 3rd 95s, strengthening the Healthy Domain, supporting nutrition services, and ensuring effective case conferencing, adherence support, and disclosure processes.
The Associate will provide programmatic and technical support to CBOs and state teams to ensure CCMP activities are implemented in line with PEPFAR guidance, CCMP Limited Waiver provisions, and national HIV treatment standards.
Key Duities and Responsibilities
Support community-based HIV case identification activities (1st 95), including household enrolment, AP3 support, and CBO-led pediatric case finding.
Support adherence, linkage, and retention interventions for CALHIV and adolescents (2nd 95), including follow-up and caregiver engagement.
Support viral load sample tracking, result follow-up, and community adherence interventions to improve viral suppression outcomes (3rd 95).
Ensure CCMP case plans and service delivery address Healthy Domain priorities, including immunization follow-up, nutrition, adherence, and linkage to essential health services.
Support nutrition screening of eligible CALHIV and households at community and facility interface levels.
Facilitate linkage to community-based or facility-based nutrition services where indicated.
Support accurate documentation of nutrition assessments and services in line with CCMP Limited Waiver requirements.
Reinforce caregiver nutrition education and infant/young child feeding (IYCF) messages during follow-up visits.
Participate in routine CCMP case conferencing meetings with state teams, clinicians, and CBOs.
Support documentation of action points, referrals, and follow-up plans for complex pediatric and adolescent cases.
Track resolution of adherence, disclosure, psychosocial, and health-related challenges discussed during case conferencing.
Support mentoring and routine engagement of CBO staff and caseworkers to improve service quality.
Assist in reviewing CBO monthly program reports and flagging gaps for corrective action.
Support coordination between CBOs, health facilities, and state CCMP teams to strengthen service integration.
Participate in supportive supervision, monitoring visits, and DQI activities as assigned.
Provide day-to-day implementation support to Kaduna State CCMP team and supported CBOs.
Support tracking of state-level CCMP workplan activities and performance indicators.
Ensure accurate documentation of beneficiary services, referrals, and success stories.
Participate in state review meetings, data validation sessions, and program performance discussions.
Required Qualificationd
Bachelor’s Degree in Social Sciences, Health Sciences, Education, or related field
Minimum of 2 years’ experience in CCMP, pediatric/adolescent HIV, or PEPFAR-supported programming
Basic computer skills, including use of Microsoft Office tools
Good written and verbal communication skills
Ability to work effectively with CBOs, health workers, and state teams
Willingness to travel within Kaduna State as required.
